Receive a Telehealth Intake Form

Fill out the physician intake form sent to you via email. It takes 2-4 days to assess your profile and issue a prescription for off-label CGM use. If we cannot provide the prescription, you'll receive a full refund.

Purchase the CGM at your local pharmacy

With a CGM prescription, you can purchase the device at your local pharmacy using a GoodRx coupon or a manufacturer's voucher for a free CGM. Most people pay around $70 to $80 for two sensors (a one-month supply)
